DIRECTIONS:

The Orthopaedic Institute of Mid-Michigan is located at 555 W. Wackerly Street in Midland, Michigan. Take US 10 to the Eastman Road exit. Take Eastman Road south to Wackerly Street (Wackerly is the first intersection just south of US 10). Turn left (east) on Wackerly Street and proceed approximately 1 mile to our facility on the right hand side.

WHAT TO BRING ON YOUR FIRST VISIT:

In order to help your physician learn as much as possible about your condition, we ask that first time patients please bring the following materials:

X-rays

MRI films/reports

a complete list of medications including dosage and frequency

any other pertient medical information

your insurance information

a referral if you are using an HMO insurance plan

any Worker's Compensation or auto claim information

OFFICE HOURS:

The Orthopaedic Institute of Mid-Michigan is open weekdays during regular business hours. Patients are seen by appointment only. Please call the orthopaedic surgeon of your choice.

INSURANCE:

We participate with many insurance plans. Please call to check on coverage with your particular plan. We are willing to discuss your insurnance coverage and fees with you. Please bring your insurance card for each visit.
